Women’s Health and Menopause Center is adopting EPIC software to hold patient electronic medical records (EMR) starting June 7th, 2021.
Setting up a Henry Ford Hospital My Chart will give you access to your labs, imaging reports and notes. It will also give you the ability to contact your physician with questions, as well as manage your scheduled visits.

To sign up you need an activation code:
- Go to the MyChart Request an Activation Code page to get your activation code.
- Once you receive an activation code, go to the MyChart Signup page to create your account.
