<p class="wp-block-paragraph">Pain during sex after menopause is common, but it should not be treated as an inevitable part of aging. Many causes are manageable once they are correctly identified.</p>



<p class="wp-block-paragraph">Some women experience discomfort near the vaginal opening. Others feel deeper pelvic pain, burning, tearing, pressure, or soreness that lasts after intercourse.</p>



<h2 class="wp-block-heading"><strong>Vaginal and Vulvar Tissue Changes</strong></h2>



<p class="wp-block-paragraph">Lower estrogen levels can cause vaginal and vulvar tissues to become thinner, drier, and less elastic. This group of changes is often called genitourinary syndrome of menopause.</p>



<p class="wp-block-paragraph">Possible symptoms include:</p>



<ul class="wp-block-list">
<li>Vaginal dryness</li>



<li>Burning or itching</li>



<li>Pain with penetration</li>



<li>Light spotting after sex</li>



<li>Urinary urgency</li>



<li>Recurrent urinary discomfort</li>
</ul>



<p class="wp-block-paragraph">Menopause-related vaginal changes may continue or worsen without treatment, unlike hot flashes, which may decrease over time for some women.</p>



<h2 class="wp-block-heading"><strong>Pelvic Floor Muscle Tension</strong></h2>



<p class="wp-block-paragraph">The pelvic floor muscles may tighten in response to pain, stress, previous trauma, childbirth, surgery, or chronic pelvic conditions. When these muscles do not relax appropriately, penetration may become painful.</p>



<p class="wp-block-paragraph">Pelvic floor physical therapy can help some patients improve muscle coordination, reduce guarding, and regain comfort. <h2 class="wp-block-heading"><strong>Other Possible Causes</strong></h2>



<p class="wp-block-paragraph">Not every case of painful sex is caused by menopause. An evaluation may identify:</p>



<ul class="wp-block-list">
<li>Yeast or bacterial infection</li>



<li>Vulvar skin conditions</li>



<li>Endometriosis</li>



<li>Pelvic organ prolapse</li>



<li>Ovarian cysts</li>



<li>Fibroids</li>



<li>Scar tissue</li>



<li>Medication-related dryness</li>



<li>Relationship or emotional factors</li>
</ul>



<p class="wp-block-paragraph">New bleeding after sex should be reported, especially after menopause.</p>



<h2 class="wp-block-heading"><strong>Treatment Options</strong></h2>



<p class="wp-block-paragraph">Treatment should match the cause. Recommendations may include:</p>



<ul class="wp-block-list">
<li>Water-based or silicone-based lubricant</li>



<li>Regular vaginal moisturizer</li>



<li>Low-dose vaginal estrogen</li>



<li>Prescription nonestrogen vaginal treatment</li>



<li>Pelvic floor physical therapy</li>



<li>Treatment for infection or a skin condition</li>



<li>Changes in sexual positioning or pacing</li>



<li>Counseling or sex therapy when appropriate</li>
</ul>



<p class="wp-block-paragraph">Systemic hormone therapy may help some women who also experience hot flashes and other whole-body menopause symptoms. Local vaginal treatment is often used when vaginal or urinary concerns are the primary issue. ACOG recognizes vaginal dryness and pain during sex as common menopause symptoms that may improve with appropriate hormone therapy or other treatments.</p>



<h2 class="wp-block-heading"><strong>You Do Not Need to Push Through Pain</strong></h2>



<p class="wp-block-paragraph">Continuing intercourse through pain can increase muscle guarding and anxiety. It is reasonable to pause, use additional support, and seek medical guidance.</p>



<p class="wp-block-paragraph">A compassionate evaluation should allow you to describe symptoms without embarrassment. Your provider may perform a gentle examination and discuss options based on your comfort level.</p>
